Hi! I am Clare and I am in my penultimate year of medical school at UNSW! At medical school, we are taught that care should go beyond the hospital — helping patients after discharge is also a critical part of healthcare. As I am constantly exposed to the more acute, medical side of things at the hospital, I believe that it is important for me to also learn how to deliver transitional/community care. Much work often goes behind caring for the vulnerable at home, and this can take a huge toll on their loved ones thus I find it meaningful to be able to contribute in any way possible. My clinical experience will enable me to educate patients and their families about their conditions, medications and post-discharge care instructions. I will also be able to advocate for a patient’s needs and preferences, ensuring they receive great care and support during the transition from hospital to home or other care settings. I like to go to the gym, go for runs along the beach and bake bread. I am an aspiring geriatrician!
