To the Employer, Hello there, my name is John Mabior, and I am interested in working at your company as I have experience to perform the job. I am applying for the Disability Support Worker position I found online. I have a real desire to help improve the quality of life for people with disability as much as possible, so they can maintain a sense of dignity in this difficult time in their life. I can help maintain a safe environment for these clients and provide the care they need regardless of their disabilities. I can also make sure they receive their medication and take it on time I have experience working in other companies in the same industry. I have experience working with people with various disabilities as I have been working with registered NDIS companies such as Bj Care Services, Kind and Care, Easy Life Home Care which had client with different types of disabilities such as intellectual, speech, mental illness, bed-ridden clients and those that are confined to a wheelchair. I am physically fit and able to help these clients in and out of bed and to help them perform other duties that require more strength or stamina than the client is capable of. I have also cared for clients that can move around on their own but need support with supervision and daily tasks such as cleaning, shopping, community access or activities, medications and meals preparation. I have also supported client with personal care such as shower, brushing their teeth and going to toilet. I have experience with doing sleepovers or active night either with two clients or one on one and at client home or at a facility centre. I also have experience with supporting clients with maintenance of a social life by encouraging them to get out and attend community functions and I can provide the transportation to these events. I can also make sure clients get to their appointments on time and back home again safely. I also have experience giving clients their medication and making sure they have taken it by observing and documenting the time, dates and any outcome. In addition to supporting people with disability, I have full driver’s license, certificate iv in disability, first Aid, CPR, working with children check, NDIS screening check and my own reliable transport to go to either client’s homes or care facility. Since I have been working in the disability industry for a while now I have knowledge to perform the administrative tasks associated with the clients such as keeping up with appointments, writing daily report of what, where, when, why, how the client have done in the day, calling emergency services if required, talking to NDIS, child safety, parents or guardian when needed or when instructed by management and recording all visits to be filed for future references. Beside the above information I have the followings ready and valid. • Victorian full driving license • Working with children check (employee) • NDIS worker’s screening check • Manual Handling • Full working rights in Australia • Clean national check • Cert IV in disability and experience • Car will full insurance • First Aid and CPR ^& ^&- I will try one of my activities with them, for example i will take them to a local park or track for short walk and if they like it and its ok with their ndis plan then i may implement walking or joking as part of their activities.^&
