Over the past 11 years, I have gained experience in providing support to individuals, families and groups as demonstrated in positions including Advanced Disability Development Support Officer and previously as House Supervisor & Team Leader, Disability Accommodations at the Department of Health & Human Services. Moreover I currently work as a School Teacher with the Victorian School of Languages and previously contributed as a Case Manager. Maintaining a strong focus and motivation, I am resilient, optimistic and persistent when resolving barriers to achieve positive outcomes. Self-motivated, dedicated and ethical, I assume full accountability for managing my own practice and professional relationships with others. I advocated for individuals to improve their quality of life, coordinated and monitored service delivery, including assuring services are delivered as described in the Individualised Service Plan. I developed and prepared a wide range of individual service plans in support of clients, ensuring plans with client’s interests in mind. Furthermore, I managed the preparation of reports regarding various incidents, including situations regarding neglect, abuse and exploitation, maintained the management and accurate preparation of progress notes, related reports and other project documentation and facilitated the completion of client and house budgeting and expenditure and progress notes, maintaining accurate and efficiency.^& ^&I possess an in-depth understanding of disability practice and the impact on the individuals their families and carers, with an awareness of relevant frameworks to respond to individual with disability, families, networks, and community. Moreover, I possess a knowledge of the theoretical and practical aspects of responding to their need, including state-level, national and international approaches to achieving their NDIS &life goals, protecting, and safeguarding their rights. Furthermore, I understand relevant legislation and practice approaches in disability, and out-of-home care, including previous and current efforts to reform the disability sector.Promoting a strong client service focus and a commitment among others to providing quality client service, I am adept in managing a high level customer service delivery and ensure client satisfaction and expectations are given high priority. Currently in my role as a NDIS Support coordinator, previously as a team leader DHHS, Victoria, I provided support for people with disability and their families from various cultural backgrounds, meeting their various demands and requirements and referred them to service providers, liaised with various stakeholders to meet permanency plan objectives.^&
